Morbus Krabbe (KRB)

Der Morbus Krabbe (Galaktocerebrosidase-Mangel) ist eine seltene, autosomal-rezessiv vererbte lysosomale Speicherkrankheit, die durch pathogene Varianten im GALC-Gen und verursacht wird und zu einer Demyelinisierung des zentralen und peripheren Nervensystems führt. Klinisch zeigt sich ein breites Spektrum, wobei die häufigste Form im Säuglingsalter beginnt und initial durch Reizbarkeit, Fütterstörungen, spastische Paresen und Entwicklungsverzögerung gekennzeichnet ist, gefolgt von rascher neurologischer Verschlechterung mit Verlust erworbener Fähigkeiten, Krampfanfällen und frühzeitigem Tod. Später einsetzende Formen verlaufen variabel, langsam progredient mit motorischen Defiziten, Sehstörungen und kognitiven Einschränkungen.
Therapeutisch steht bei asymptomatischen oder früh diagnostizierten Patienten die hämatopoetische Stammzelltransplantation im Vordergrund, während bei symptomatischen Patienten überwiegend supportive Maßnahmen zur Anwendung kommen, einschließlich symptomatischer Behandlung neurologischer und systemischer Komplikationen.
Die Prävalenz liegt in Europa bei etwa 1:100.000 Geburten, mit regionalen Schwankungen.

Die Diagnostik erfolgt durch den Nachweis einer stark verminderten Enzymaktivität in Leukozyten oder Fibroblasten ergänzt durch molekulargenetische Analysen.

Die Kosten werden bei bestehender medizinischer Indikation über einen Überweisungsschein Typ 10 (EBM) abgerechnet. Humangenetische Leistungen sind nicht budgetrelevant. Für privatversicherte Patienten sowie private Kostenträger (Krankenhäuser etc.) können auf Wunsch entsprechende Kostenvoranschläge erstellt werden.
