Library: Capture based TWIST Library Sequenzierung: Aviti, Illumina- NovaSeq6000 oder NovaseqX Sequencer Variantenklassifikation gemäß ACMG-Guidelines (Richards et al. 2015, PMID: 25741868) sowie, falls verfügbar, Gen-spezifischer Guidelines (https://cspec.genome.network/cspec/ui/svi) Varianten unklarer Signifikanz (VUS) werden nur berichtet, wenn sie als klinisch relevant angesehen werden Punktesystem nach Tavtigian et al., 2020, PMID: 32720330): „hot“ VUS: 5 Punkte; „warm“ VUS: 4 Punkte; „tepid“ VUS: 3 Punkte; „cool“ VUS: 2 Punkte; „cold“ VUS: 1 Punkt; „ice cold“ VUS: 0 Punkte). Bestätigung des NGS-Ergebnisses wird für krankheitsauslösende Varianten mittels konventioneller Sanger-Sequenzierung durchgeführt, sofern die internen Qualitätskriterien nicht erfüllt sind. Limitationen: Varianten (SNV/InDels) werden ab einer Allelfrequenz von >25% nachgewiesen. Größere Insertionen/Deletionen oder InDels können unter Umständen nicht detektiert werden, da sie als nicht-passende Bereiche vom Mapping-Algorithmus entfernt werden. Nicht alle potentiell relevanten Bereiche der untersuchten Gene (z. B. regulatorische Bereiche, Introns) können komplett abgedeckt werden. Strukturaberration oder Keimbahnmosaike können nicht ausgeschlossen werden. Bereiche mit schwierigen Sequenzbereichen wie z.B. repetitiven Sequenzen oder homologen Bereichen (LINEs, SINEs, Alu-Elemente, Pseudogenen oder Trinukleotid-Repeats) mit deutlich eingeschränkten Nachweisgrenze.
Kosten
Die Kosten werden bei bestehender medizinischer Indikation über einen Überweisungsschein Typ 10 (EBM) abgerechnet. Humangenetische Leistungen sind nicht budgetrelevant. Für privatversicherte Patienten sowie private Kostenträger (Krankenhäuser etc.) können auf Wunsch entsprechende Kostenvoranschläge erstellt werden.
